Summary

Requires a health care provider, when providing treatment to a minor for alcoholism, alcohol abuse, drug abuse, or a venereal disease, to make a reasonable effort to contact the minor's parent or guardian to provide information on the minor's condition and proposed treatment and to document in writing each attempt the health care provider makes to contact the parent or guardian of the minor.
